The Czech-born director died in the USA after a brief illness, his wife, Martina, told Czech news agency CTK, according to The Hollywood Reporter.

Jan Tomas Forman is born in Caslav in the former Czechoslovakia.

In 1968, he moved from what was then Czechoslovakia to the United Sates after Warsaw Pact troops invaded the country to crush the "Prague Spring".

Forman recalled the last time he saw his father: when aged seven he was pulled out of class to see Forman senior escorted by two leather-coated members of the Gestapo.

"In his films you can see the author", he said.

His career overseas started with "Taking Off" in 1971, followed by "One Flew Over the Cuckoo's Nest" four years later, which brought him his first "best director" Oscar.

He became an established foreign film director in the 1960s for "Loves of a Blonde" and "The Firemen's Ball", which were nominated for Oscars in the best foreign-language film category.

Married three times, Forman met his third wife Martina - a writer three decades his junior - in Prague in the 1990s.

With some trepidation, Forman returned to Czechoslovakia to make "Amadeus" because Prague was one of the few European cities that had been little changed since Mozart's time.

Actor Michael Douglas gave Forman a second chance, hiring him to direct One Flew Over The Cuckoo's Nest, which Douglas was co-producing.

He was raised an orphan after both his parents died in concentration camps during World War Two.

Never prolific, Forman's output slowed even more after Amadeus, and his three subsequent films were disappointments.

"The Communist Party was my Nurse Ratched, telling me what I could and could not do", he said about the despotic main female character.

Larry Karaszewski, who co-wrote Man on the Moon and The People vs Larry Flynt with Scott Alexander, called Forman "our friend and our teacher" on Twitter.

The director's first marriage, to actress Jana Brejchova ended in divorce.

However, that wasn't the end of it as Forman went on to win his second Oscar for the 1984 Hollywood film "Amadeus", a biopic of Wolfgang Amadeus Mozart. They were named after comedian Andy Kaufman and Jim Carrey, who played the part of Kaufman in "Man on the Moon". They have twin boys.
